November 4, 2005	Deadline for submission of Committee Assignment Form 2006. New RAC Bylaws require that each member prepresentative choose a primary Committee and attend one half of that Committee's meetings in order to maintain RAC eligibility. Click here to access blank form.
November 4, 2005	Deadline for submission of Hospital Needs Assesment to the RAC Office. These documents must be mailed to the RAC office via certified mail return receipt requested as these documents are required to maintain your RAC eligibility. Click here to access blank form.

December 31, 2004	Deadline for Disbursement of Final Payments to Participating Hospitals in HRSA Bioterrorism Program. Witheld 25% of funds will be disbursed by RAC-R Executive Director to all hospitals who submitted complete invoices accounting for the expenditure of all money awarded by the December 15, 2004 deadline. July 20, 2004	RAC-R Grant Funding (Tobacco Contract) Reports accounting for purchases with paid receipts must be in the hands of the Executive Director. June 30, 2004	Extended Deadline for Tier One Bioterrorism Funds to be expended and accounted for.
